Stock Transfer
Gifting stock is one of the many ways you can support the Foundation. Please follow the steps and complete the form below to facilitate your gift. We sincerely appreciate your generosity and can help if needed.

Step 1: Initiate the Stock Transfer
Securities can be gifted to the Stephen A. Comunale Jr. Family Cancer Foundation in two ways:
- Electronic transfer (in which your broker transfers shares using the Stephen A. Comunale Jr. Family Cancer Foundation DTC number)
- Physically mailing the paper certificates to the Stephen A. Comunale Jr. Family Cancer Foundation.
Either method is acceptable, but it's necessary for you to contact your broker to initiate the transfer. If you have questions, please contact Linda O'Connell of Morgan Stanley Wealth Management at 315-448-3400.
Step 2: Notify the Stephen A. Comunale Jr. Family Cancer Foundation
Complete the electronic form below to notify us of your gift. This step is important. If you transfer stock electronically, your name will not be associated with the transaction. We need this information so we are aware the transaction is taking place and that we can properly acknowledge you for your gift.
Step 3: Send securities to the Stephen A. Comunale Jr. Family Cancer Foundation
